Before you begin. Allow approximately 20 minutes to complete this form. Have your vessel particulars (MMSI, IMO, call sign), Starlink kit serial numbers, and billing details to hand. You will be required to download both a copy of your completed form and a copy of the Master Services Agreement before you can submit. By submitting, you agree to the KVH Starlink Activation Order and the Clase Connected Master Services Agreement — read or download the full Master Services Agreement here.

Subscriber Information

Who is the subscriber? The legal entity that owns this Starlink activation. For corporate customers this is the registered company name and registered address; for personal vessels this is your name and home address.

SECTION 02

Billing Information

Required

Where invoices go. Often identical to the subscriber, in which case tick "Same as Subscriber" to copy the details across. Differs when a parent company or management company pays on behalf of the owning entity.

The first invoice will be for the activation fee. UK customers will be invited to set up a GoCardless Direct Debit mandate to handle recurring monthly invoices automatically.

SECTION 03

Authorised Contacts

At least one Onboard + one System Support

People we can talk to about this account. You must nominate at least one Onboard Contact (someone on the vessel — typically the captain or first officer) and one System Support Contact (your IT lead, may be ashore).

The same person can hold multiple roles by ticking more than one box. Contact 2 is optional but strongly recommended in case Contact 1 is unreachable.

SECTION 04

Vessel & Installation

Particulars of the vessel. MMSI, IMO, and Call Sign are mandatory for commercial vessels but may be left blank for leisure. Flag is the country of vessel registration.

Pick the vessel type from either Commercial Vessel Type or Leisure Vessel Type — leave the other set to "Not Applicable". Length helps us validate the plan size; large vessels (over 80 ft / 24 m) typically need higher-tier plans.

SECTION 05

Installer Information

Who installed the equipment. "KVH Authorised Dealer" is standard for commercial vessels. "At Factory" means the boat builder fitted it as OEM — supply the manufacturer name in that case.

"Self" installation is permitted for leisure customers but you accept responsibility for installation quality, EMI/EMC compatibility, and antenna positioning.

System Identifiers

Kit Serial preferred

Starlink KIT Serial No. is mandatory — it starts with "KIT-" and is printed on the box and visible in your Starlink account. The other identifiers are optional aids to faster activation.

If your terminal was supplied as a KVH bundle, you may also have a KVH System Serial. Otherwise leave KVH Serial blank.

SECTION 07

Airtime Plan

Select one

Pick one plan. Plans are denominated in USD and billed monthly in advance. Clase Connected invoices you in GBP at the prevailing exchange rate.

Opt-in overage means data continues at $2.25/GB once your allowance is exhausted — recommended for any commercial vessel. Opt-out means data stops when offshore or underway once the allowance is consumed.

Public IP is optional (POA) and only required if you need inbound connections from the open internet (remote management, VPN concentrators, hosted services).

1 Overages per GB are billed at $2.25. Customers MUST opt-in to overage to continue Starlink data in ocean areas and/or underway if the subscribed / allotted data has been used. If the vessel is in ocean areas and/or underway, or leaves Starlink land coverage, there will be NO Starlink data if the subscribed / allotted data has been used and plan is on Opt-out overage. If a vessel is on Opt-in whilst in a marina and exhausted the monthly Priority Data allowance, any data used outside the monthly allowance will be charged as overages.

SECTION 08

KVH OneCare

Select one

One OneCare plan is required per terminal. Essential is included free of charge and provides basic warranty coverage and email support during business hours.

Standard, Premier, and Premier+ add escalating tiers of 24/7 support, response time guarantees, and hardware replacement coverage. Premier+ is recommended for commercial vessels operating in remote locations.

A subscription-based OneCare Global Support Program is required for each terminal receiving KVH Starlink Airtime Services.

SECTION 09

Contract Details

Service Start Date is the date you want the activation to take effect. It cannot be in the past. The activation will not actually begin until the activation fee invoice is paid in cleared funds — this date is the requested start.

Promotion Code and Customer Code are optional and only apply if Clase Connected has issued you a specific code.

SECTION 10

Traffic Management & Firewalls

Strongly recommended

Modern devices (smartphones, tablets, laptops) frequently consume large volumes of data through background cloud sync, OS updates, and telemetry — often without the user's knowledge. This can lead to bill shock on metered marine connections.

We strongly recommend "Yes" — our airtime team will configure your Peplink router with sensible defaults (firewall rules, traffic shaping, bandwidth limits) to prevent runaway usage. There is no extra charge for this on managed Peplink installations.

Do you require network traffic management / firewall protection? *

Modern devices (smartphones, tablets and laptops) frequently consume large volumes of data through background cloud-based services. This can result in unnecessarily high background data consumption, often without the device user's knowledge, leading to bill shock. Our airtime team can implement custom restrictions on your Peplink router to proactively manage and limit such usage. For further information please contact [email protected].

SECTION 11

Network Usage & DPI Reports

Acknowledgement that your Peplink licence will be renewed automatically alongside your airtime service. The Peplink licence extends the hardware warranty and the validity of any traffic management, firewall, or device reporting we apply.

This is mandatory for any vessel subscribing to Clase Connected airtime. Without an active Peplink licence we cannot offer managed network services or generate monthly DPI usage reports.

We offer one complimentary network and device/application data usage report per month (upon request) during the first 3 months following network activation and commissioning. After this period, additional reports are available at £25.00 ex VAT per report, limited to a maximum of one report per month.

Please note: Our Traffic Management, Firewalls and Device Report services can only be offered to networks fully managed by Clase Connected and with active Peplink support licences. Active Peplink licences are mandatory for vessels subscribing to any of our airtime services. By signing this airtime agreement, you also agree to the mandatory renewal of these licences, which extends the warranty on your Peplink equipment.
